Type of work to be done
Type of traffic control needed (ex. Lane closure, sidewalk closure, full road closure)
Dimensions of work area
Distance from nearest intersection to start of work
Primary site contact information
Notes on access problems for adjacent properties
After-hour conditions (if work is not completed)
Specific work hours and days
